Meredith Niess, MD, MPH
Chief Medical Officer
CCHC – University & West

Dr. Niess is is a graduate of Dartmouth College in New Hampshire and the University of North Carolina School of Medicine in Chapel Hill. She completed her Internal Medicine training and a primary care research fellowship in Colorado and then was an Assistant Professor at Oregon Health and Science University where she specialized in complex Medicaid patients transitioning from hospitalized care. Prior to moving back to North Carolina, she worked at a Community Health Center in New Haven, Connecticut where she focused on care for patients with severe mental illness. She is a Charlotte native and excited to be back where she grew up and has family!

Marie Brown, MSN, FNP-BC, MPH
Assistant Medical Director
CCHC – University

Marie is bilingual in Spanish and English and board certified for family primary care through the American Nurses Credentialing Center. She earned her Bachelor’s degree in Nursing from Boston College and is a graduate of Johns Hopkins School of Nursing and School of Public Health. Prior to CCHC, she worked at Access Community Health Network as a Family Nurse Practitioner in Chicago, IL. She also worked at Georgetown Hospital and in the rural areas of Peru as a nurse. She is very passionate about community health and providing equal healthcare for all. She has particular interest in integrative medicine, nutrition, chronic disease management, and public health policy.

Hilary Canipe, MD
CCHC – University

Dr. Canipe is board certified by the American Academy of Family Physicians and has served with Charlotte Community Health Clinic since 2017. Before coming to Charlotte, she worked for 3 years in Waco, TX in private practice, then for over a decade with Roanoke Chowan Community Health Center in Murfreesboro, NC. She attended Wake Forest University both for undergraduate and medical degrees, and completed her residency at Memorial Medical Center in Savannah, GA. She enjoys improving health literacy, managing chronic disease in collaboration with patients, and encouraging preventative care.

Michelle Hewlett Wynn, MD
CCHC – University

Dr. Wynn is a graduate of Northwestern University in Evanston, Illinois and the University of North Carolina School of Medicine in Chapel Hill. She completed her residency and a chief residency in Pediatrics at the Medical College of Ohio in Toledo. She was the first pediatrician to join the practice when CCHC began to see children in March 2011. She has experience in private practice at Kaiser Permanente HMO and at C.W. Williams Community Health Center prior to coming to CCHC. Dr. Wynn is board-certified by the American Academy of Pediatrics and sees patients from birth through the end of high school. She has a special interest in promoting healthy lifestyle habits in her young patients that will hopefully last a lifetime.

Kimberly Polonsky, MSN, APRN, FNP-BC
CCHC – West

Kimberly is board certified as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center. Prior to CCHC, she worked as a FNP at Neponset Community Health Center in Boston, MA. Kimberly attended Duke University School of Nursing and practiced as a nurse in the Duke Hospital Emergency Room prior to going back to school for her Master’s degree in nursing from UNC-Chapel Hill. She also has a Bachelor’s degree in Economics from Middlebury College in Vermont. She loves all aspects of primary care and has a particular interest in chronic disease management, diabetes management, women’s health, and adolescent care.

Quiana Robinson, DDS, MS
Dental Director
CCHC – West

Dr. Robinson is a native of Charlotte, NC. She has been practicing for almost 10 years and brings a wealth of experience. She attended Winston Salem State University where she received her Bachelor of Science degree in Biology. Dr. Robinson has also earned her Doctor of Dental Surgery degree from Meharry Medical College. She has a background in hospital dentistry, working with our Veterans and has also worked in private practice. In her spare time, she enjoys working out, traveling, and spending time with her husband and their son.

Rachelle Vanessa Kazakos, RDH
CCHC – West

Rachell Vanessa Kazakos is originally from Panama and grew up in Southwest Florida. She practiced dental hygiene for 25 years in Florida. Most of her career focus was on periodontal therapy, with three board certified periodontists. In her spare time she enjoys spending time mountain biking, working out, and hiking around the Blue Ridge Parkway. She has a daughter with whom she enjoys traveling and making memories with.

Behavioral Health

Patricia (Tricia) Greer, MSW, LCSW, LCAS
CCHC – West

Tricia is a Licensed Clinical Social Worker in our West location. She attended UNC-Charlotte and earned a Bachelors of Social Work (BSW), and minors in Sociology and Religious Studies. She then earned a Masters in Social Work (MSW) and is both a Licensed Clinical Social Worker (LCSW) and a Licensed Clinical Addiction Specialist (LCAS). She firmly believes each individual in treatment can learn, grow, heal and/or better manage their symptoms by expressing their feelings appropriately, changing how they view their lives, and learning how to better manage their issues to find more balance in life. She encourages people to be comfortable in their own skin and find their own voice.

Yovani Sanchez Rivas, MSW, LCSW
CCHC – University

Yovani is a Bilingual Licensed Clinical Social Worker and sees patients in our University location. She holds a Master’s Degree in Clinical Social Work from the University of Wyoming. As a therapist, she uses mindfulness techniques to help her patients that suffer from anxiety, depression, and other mental conditions. Yovani moved to Charlotte 4 years ago. When she is not working, Yovani likes to spend time playing with her two daughters, as well as walking her new puppy.
