Children’s Health Day

Charlotte Community Health Clinic is excited to once again be hosting Children’s Health Day on Friday, August 9th from 9 am – 3 pm in celebration of National Health Center Week. The day will include back-to-school physicals for school-aged children, as well as activities, snacks, and a free backpack full of school supplies for each child who receives a physical. CCHC accepts Medicaid and most major insurance plans. Uninsured patients will be responsible for a $25 copay, which includes both the doctor visit and any necessary vaccinations.

Appointments are required, and we have a limited number of appointments available, so please follow the steps to schedule a physical:

  1. Register your child by calling the clinic at 704-316-6561.
  2. Fill out the patient application if this is the first time your child has visited a doctor at CCHC. You can download our application in English here, and in Spanish here.
  3. Email your completed application to or bring a printed copy of your completed application into the clinic before Tuesday, August 6th. If we do not receive your completed application by August 6th, your appointment will be canceled.
  4. A CCHC staff member will call to let you know we have received your application and confirmed your previously-scheduled appointment.
  5. Come to the clinic on August 9th to receive your physical and a free backpack full of back-to-school goodies!
